Billing Change Request Form
Please read carefully before proceeding:
- Cancellation requests must be received by the 25th of the month to be in effect for the upcoming month.
- Freeze requests must be received by the 25th of the month to be in effect for the upcoming month.
- Memberships can be frozen for $12.00 per month per member*.
- Monthly freeze fees may be waived for medical reasons; medical documentation must be sent to [email protected] before the 1st of the month.
- If your membership is frozen for medical reasons; you will not be permitted to use the facilities until your membership is either unfrozen or terminated.
- Please come into the gym to unfreeze your account in person on your first visit back.
- Please allow up to 5 business days for any response to your billing inquiry.
For Youth Program Members:
- Cancellation requests must be submitted one calendar month before you wish to leave the team (e.g., if you wish to leave in March, you must submit your request before February 1st).
- Unless you have a medical reason (see above for policy on medical freezes), we do not approve freezes. However, if you wish to rejoin the team after canceling, the enrollment fee is reduced to $39.
